Comments:
An unannounced non-mandated inspection took place on November 20, 2019, beginning at approximately 2:50 PM and ending at approximately 4:50 PM. Staff records were reviewed at a separate site. At this site location, five children's records were reviewed during the inspection. There were ten children in care and two staff present. There were no medications onsite during this inspection. All areas that are used for care were observed. The inspector looked at first aid and emergency supplies, drill documentation, and required health and fire inspections. The children and staff were observed during the following activities: arrival, snack time, free play, and homework time. There were discussions about the children's records, licensing and inspection processes, and hazardous substances. The violations were discussed at the conclusion of this inspection. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion with staff, the center failed to ensure that the separate record for each child enrolled contained all the elements as required by the standards.

Evidence:

1. Records for Child 1,2,3,4 and 5 were missing the home address of each parent who has custody;
2. Records for Child 2 and 3 were missing previous child day care and schools attended by the child;

Plan of Correction: Staff will correct the missing information within 30 days and site director will make sure all needed information is in the file the day of enrollment.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children. If a key is used, the key shall not be accessible to the children.

Evidence:

1. Within the cafeteria, where the children were in care, there were two spray bottles of "fabric fire protection" in an unlocked cabinet, a spray bottle of "ready to use disinfectant" located on top of a cabinet, a box of 4, 1 gallon bottles of floor cleaner, and several gallon bottles of window cleaner. All of these substances were located at eye level of a small child or located on the floor of the cafeteria. All substances listed a "keep out of reach of children" warning.

Plan of Correction: Staff will make sure before the children are released daily that all chemicals are locked away even the schools supplies.

Description: Based on observation, the center failed to ensure that cleaning and sanitizing materials shall not be located above food, food equipment, utensils or single-service articles and shall be stored in areas physically separate from food.

Evidence:

1. A "fabric fire protection spay" bottle and a bottle of spray disinfectant were stored in a cabinet, located in the cafeteria, with chocolate covered coffee beans, canned corn, paper plates, and styrofoam "to go" containers. The spray bottles were located on the top shelf and the food and single-service articles were stored on the shelf below. All substances listed a "keep out of reach of children" warning.

Plan of Correction: Staff will make sure before the children are released daily that all chemicals are locked away even the schools supplies.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center failed to ensure that electrical outlets shall have protective covers that are of a size that cannot be swallowed by children.

Evidence:

While children were playing in the gym and while a preschool age child was in care, the LI observed five electrical outlets without protective covers.
