Comments:
A Subsidy Health and Safety inspection was conducted on November 18, 2019 to review supplemental health and safety requirements. Eight staff were present with thirty-nine children. Five children and five staff records were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Please complete the columns for ?description of action to be taken? and ?date to be corrected? for each violation cited on the violation notice, and then return a signed copy to the licensing office within 5 calendar days of receipt. Description: Based on review, facility did not ensure 1 of 5 staff have documentation of completed Virginia Preservice Training for Child Care Staff within 90 days of beginning employment.

Evidence: The record for staff #5 (start date August 19, 2019) did not have documentation of compelted Virginia Preservice Training. Staff #2 stated the training has not been done.

Description: Based on interview, facility did not ensure there is documentation of all required written policies and procedures.

Evidence: There was not a written policy and procedures that describes how they will ensure that each group of children receives care by consistent staff or team of staff members.
