An unannounced renewal inspection was completed on 12/02/2019. There were 43 children, ages 2 years-5 years, and 8 staff members present during the inspection. The children were observed in the following activities: during story time, during free choice/center play, in large motor play in the gym, during lunch and during naptime. The inspector reviewed 5 children's records, 5 staff records and 1 medication during the inspection. The inspector reviewed the emergency preparedness plans, the parent handbook and the emergency supplies during the inspection. The inspector discussed the following with the staff: medication, cleaning supplies, and nutrition. Standard #: 22VAC40-185-60-A Description: Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled contained all of the elements as required by the standards.
Evidence: The record for Child #2 did not contain the health information as required by 22VAC40-185-130 and 22VAC40-185-140; the information was in the record but did not meet the time frame requirements.Plan of Correction: The information will be obtained within the time frame required.
Standard #: 22VAC40-185-510-N Description: Based on medication review and discussion with the staff, the facility failed to ensure that when an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.
Evidence: The authorization form for Medication A had an expiration date of 11/11/2019; the medication was still on site during the 12/02/2019 inspection.Plan of Correction: The Director will return the medication to the parent and will ensure that the forms are properly filled out in the future.
