An unannounced renewal inspection was conducted from 8:45 am-11:15. There were a total of 188 children with 27 staff members present. Children were observed during snack time, engaged in individual activities and outdoor play time. A selection of staff and children records, medications and other documents were reviewed. Standard #: 22VAC40-185-160-C Description: Based on a review of staff records, at least every two years from the date of the last screening, staff have not submitted results of a follow up tuberculosis (TB) screening.

Standard #: 22VAC40-185-510-C Description: Based on a review of children medications, long-term prescription drug use and over-the-counter medication does not have written authorization from the child's physician and parent. The center is not following the procedures to prevent use of outdated medication.
Evidence: Child #3 medication authorization form signed and dated by the physician on 8/23/19 does not have written authorization from the parent.
