Technical Assistance:
Discussed the required documentation for children enrolled in your new CASA Academy on site.
Please forward your renewal application and licensing fee prior to your expiration date.

Comments:
An unannounced Renewal inspection was conducted this date. A total of 21 children and 4 staff were observed in the classrooms. Observation of programming in each classroom was made. In the younger class, the children were seated on chairs in a circle while the teacher reviewed letters, names of the children and colors. Staff were encouraging the children to participate and sound out the words. In the older class, bathroom and hand washing procedures were observed as they transitioned from circle time to snack time. Both classrooms were decorated with hand print fall leaves and tissue paper paint artwork. Observation of the playground was made and the grounds around the school. No medications at this time and no identified food allergies. A total of 5 children's files and 5 staff files were reviewed. CPR/First Aid, MAT and DOH training was verified. All staff files to include background checks and qualifications are reviewed annually at the Administrative office. Thank you for your cooperation today. Standard #: 22VAC40-185-140-A
Description: Based on child record review, a health exam was not maintained for one child.
Evidence: Child #3 attends the CASA Academy. A physical exam was not maintained for this child.

Plan of Correction: We will obtain a copy of the physical exam for this child.

Standard #: 22VAC40-185-70-A
Description: Based on review of staff records, 2 out of 5 staff records reviewed were incomplete.
Evidence: Staff #3 and 5 did not have emergency contacts on file.

Plan of Correction: This information is saved electronically and we are unable to access this system at this time. We will have each staff document an emergency contact and we will maintain this information.We will have this completed today.

Standard #: 22VAC40-185-280-B
Description: Based on observation of the CASA Academy, hazardous substances were not locked.
Evidence: 3 cans of adhesive spray was found in a cabinet in the CASA Academy room unlocked. These were labeled "keep out of reach of children".

Plan of Correction: We will secure this cabinet immediately.

Standard #: 22VAC40-185-330-B
Description: Based on observation of the playground area, resilient surfacing was not maintained.
Evidence: The resilient surface areas by the fall zones did not maintain at least six inches of mulch.

Plan of Correction: We will email County maintenance to rake the mulch to cover these areas.

Standard #: 22VAC40-185-550-M
Description: Based on review of the written record of injuries, the record did not include all required documentation.
Evidence: Injury records for 3/26/19, 6/26/19 and 3/15/19 did not include either a staff and parent signature or two staff signatures.

Plan of Correction: We will ensure that all parents sign the injury record or 2 staff will sign.
