Technical Assistance:
Technical assistance was offered regarding obtaining required children's paperwork before first day of attendance.

Comments:
An unannounced inspection was conducted on 11/22/19 from 10:40 am to 11:50 am, in response to a complaint received on 11/20/19. The complaint was related to a child being permitted to attend the center without required paperwork for the child's record. 3 staff were interviewed and one child's record was reviewed. The center's online system to record children's information was also reviewed. Based on the preponderance of evidence, the complaint is determined to be valid. Complaint related: Yes
Description: Based on record review, the center did not obtain immunization records for children in care before the first day of enrollment.
Evidence:
1. Child # 1 first day of attendance at the center was 11/18/19.
2. Child # 1's record was missing the immunization record, which was not obtained by the center before the first day of attendance.

Plan of Correction: All staff will verify each child on roll call sheets to ensure all records are completed for each child.

Description: Based on records review, the center did not maintain a complete record for children in care.
Evidence: Child # 1's record was missing the information for two emergency contacts.

Plan of Correction: No staff will take any child without verifying proper documentation. Roll call attendance sheets.
