Standard #: 22VAC40-185-130-A Description: Based on record review, the center did not obtain immunization records for children in care before the first day of enrollment.
Evidence:
1. Child # 1 first day of attendance at the center was 11/18/19.
2. Child # 1's record was missing the immunization record, which was not obtained by the center before the first day of attendance.Plan of Correction: All staff will verify each child on roll call sheets to ensure all records are completed for each child.
Standard #: 22VAC40-185-60-A Description: Based on records review, the center did not maintain a complete record for children in care.
Evidence: Child # 1's record was missing the information for two emergency contacts.Plan of Correction: No staff will take any child without verifying proper documentation. Roll call attendance sheets.
