Comments:
An unannounced monitoring inspection was completed on 12/3/19. There were 55 children present. Five children's records and five staff records were reviewed. Four emergency medications were reviewed for three children in care.

The inspector arrived at the center at 2:00 pm and departed at 4:15 pm.

The children were observed during rest time, during outside time, during afternoon snack, during free choice time in the classrooms, and during pick up.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that each child's record contained documentation of all required information.

Evidence:
1. The record for child #2 was missing the address for one of two required emergency contacts. The name, address, and phone number of a person to contact in an emergency if the parents can not be reached is required.

2. The record for child #4 was missing a complete address for two of two required emergency contacts. The city only was listed for both contact's addresses.

Plan of Correction: The assistant director will contact the parents to obtain complete addresses for the emergency contacts.

Standard #: 22VAC40-185-280-G
Description: Based on observation, the center failed to ensure that hazardous substances not kept in original containers were clearly labeled to indicate their contents.

Evidence:
1. There were two small generic spray bottles observed to be used by staff to clean the tables. Staff were asked the contents and stated that there was soap and water in the spray bottles. The spray bottles were not clearly labeled to indicate the contents in the spray bottle.

Plan of Correction: This was corrected during the inspection. The spray bottles were labeled with the contents.
