Technical Assistance:
Technical assistance today included storage of medications, medication authorizations and record keeping. For information on training for your new administrative assistant, the phone number at our office is 703 934-1505.

Comments:
An unannounced monitoring inspection took place today from 1030 am - 1:30 pm. There were 25 child present to celebrate a Thanksgiving lunch with staff and parents. I observed children preparing for their lunch, after lunch conversation, and preparing for naptime. The rooms were filled with recent cooperative artwork efforts and seasonal art. Description: Based on a review of a sample of children's records, there were some items missing.
Evidence:
Child # 4 had two emergency contacts but no address as required. Child # 3 (start date: 7/19) did not have proof of identification documented within 7 says of beginning as required.

Plan of Correction: I will contact the parents and get this information as soon as possible.

Standard #: 22VAC40-185-510-E
Description: A bottle of medication was observed in the medicine box without a label as required.
Evidence:
A container of antihistamine did not have the name of the child or dosage as required.

Description: Based on a review of medications and authorization today, not all met requirements to administer.
Evidence:
The medications for Child # 5, 6 and 7 did not have documentation of long term authorizations as required for long term medication. Child # 6 had an emergency allergy medication on site since the beginning of school but no physician's authorization as required.Child # 7 had no authorization available for review. The emergency allergy medication for Child # 6 had an expiration date of 10/19. medication

Children's medications were stored today in a high cabinet in the office but no locking mechanism was available as required.

Description: Based on a review of children's medications today, there was a medication that was present that was no longer required.
Evidence::
The authorization for the medication for Child # 5 was no longer authorized after July, but was still present in the medication box..
