Comments:
An unannounced focused inspection was completed on November 7, 2019 from approximately 3:00 p.m. to 4:30 p.m. to review compliance of specific standards. Twenty-four children were in care and three staff were on premises during the inspection. Staff and children were observed in the indoor space, engaged in activities to include arrival and departure from the program, completing homework, and having snack. Interviews and interactions were conducted and five children?s records and two staff records were reviewed. Medications are not given and transportation is not provided. The administrator was available for the inspection and present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of children?s records, the licensee failed to maintain all the elements of a child?s record for four of five children.
Evidence: 1. Child #1 and Child # 5's records lacked information pertaining to any chronic physical problems and pertinent developmental information. Child #2 and Child #3's records contained only one of two required two emergency contacts. 2. Administrator #1 acknowledged this information was not documented in these children's records.

Plan of Correction: The children''s parents will be contacted by after school staff to obtain an additional emergency contact and the owner will add the missing children's record elements to the electronic app so there is a place to capture this information at registration if it is pertinent.

Standard #: 22VAC40-185-500-A
Description: Based on observation and interview, the licensee failed to ensure that children's hands shall be washed with soap and running water or disposable wipes before and after eating meals or snacks.
Evidence: 1. The children were observed arriving to the program after school. After being signed in many of the children began to eat their snack. The children were not observed washing their hands prior or using disposable wipes. 2. In an interview with Administrator #1, she reported that there is not currently a process in place for this as the children wash their hands at school right before arriving to the after school program.

Plan of Correction: The owner will come up with a hand washing process before and after snack.
