Comments:
An unannounced monitoring inspection was conducted on Thursday, November 21, 2019 to determine the center?s compliance with licensing standards. The inspector arrived at 10:15am and concluded the inspection at approximately 1:45pm. The center?s director and owner were present and assisted the inspector throughout the inspection. The census for today?s inspection consisted of 58 children in the direct care of 12 staff members. Additional staff were on site and available to the children in care. Upon arrival, the children were engaged in a variety of activities. Each classroom was inspected and formal observations were completed. The center is equipped with age and stage appropriate materials and equipment for the children?s use. Staff were engaged with the children and were observed offering guidance when needed. All areas where the children receive care were inspected and found to be in compliance. The center had the following posted: license, daily schedules, emergency numbers, evacuation maps, menu, and various parent information. Today?s lunch consisted of turkey and cheese sandwich, fruit, peas, and milk. The center?s first aid kit and non-emergency supplies were inspected and found complete. During the inspection, five children?s records and five staff records were reviewed. Medication is administered and one emergency medication was reviewed today. The most recent evacuation drill was documented on 11/15/2019. Two shelter-in-place drills have been documented: 06/20/2019 and 11/20/2019.

Standard #: 22VAC40-185-70-A
Description: Based on a review of five staff records on 11/21/2019, the center did not ensure that one staff's record contained the required information.

Evidence: 1) The licensing inspector observed two references in the record for Staff #5 (DOH: 08/16/2019). The references were taken over the telephone, but there was no documentation of the dates of contact with each reference. Each staff record shall include documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering. If a reference check is taken over the phone, documentation shall include dates of contact, names of persons contacted, the firms contacted, results, and signature of person making call.

Plan of Correction: Per the Center: Moving forward we will double check dates are on each reference on the day of the phone call.

Description: Based on a review of documentation on 11/21/2019, the center did not ensure that injury records contained the required information.

Evidence: 1) The licensing inspector observed that five out of five injury records reviewed did not contain documentation showing how parents were notified of the injury; or the date and time parents were notified of the injury.

Plan of Correction: Per the Center: Teachers will be given another training on how to fill out injury reports properly.
