Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, December 4, 2019. The on-site inspection was from 10:25 a.m. to 1:50 p.m. On this date, there were 23 children present in three classrooms and in the care of five staff members. There was one additional staff member available to assist if needed. There were 36 children enrolled in the center. Five staff members' records and five children's records were reviewed. Three classrooms were toured and observations were made in two of those classrooms. Six staff members and three children were interviewed. The medication and medication paperwork were reviewed for one child. Lunch was brought from home and the menu for afternoon snack, provided by the center, was posted. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. Some items reviewed/observed: transitions to and from the playground, infant feeding, story time, diaper changing procedures, bathroom and hand washing procedures, lunch, songs, language art activities, nap time, annual health and fire inspections, injury reports, documentation of evacuation, shelter-in-place drills and lock down drills, first aid kit and emergency supplies, warm water temperature, and required posted information.

Violations:
Standard #: 22VAC40-185-440-B
Description: Based on observation and verification from staff, two cribs were not labeled for use by specific children.

Evidence:
1. There were two cribs which were not labeled with the names of the children using them.
2. Staff A confirmed whose cribs they were and that they were not labeled.

Plan of Correction: The cribs were labeled immediately during the inspection.
