Comments:
An unannounced Monitoring Inspection was conducted today from 11am to 11:35am (class went to cafeteria). There were 31 preschoolers directly supervised by 5 staff. The physical plant, 4 children?s records, no children's medications and medication authorization records were on site, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in "centers", listening to stories, playing with puzzles,interacting with staff, and preparing for lunch. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Description: Based on observation, the facility did not ensure that cleaning and sanitizing materials shall not be located above food, food equipment, utensils or single-service articles, and shall be stored in areas physically separate from food.
Evidence: Observed dish soap stored above food for children in care.

Description: Based on review, the facility did not ensure that they practiced evacuation drills monthly.
Evidence: Reviewed the evacuation drill log and found that there was no evacuation drill conducted in October.
