A Subsidy Health and Safety inspection was conducted on 11/20/2019 to review supplemental health and safety requirements. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection.
Violations were found during the supplemental subsidy health and safety inspection and are listed on the violation notice.
Ninety-eight children are present with 15 staff, an administrator and cook.

Standard #: 22VAC40-665-580-B Description: Based on the review of six staff files, the facility failed to ensure that all staff have completed VA Preservice training within 90 days of employment.
Evidence:
1. Staff # 3 has not completed the preservice training. Hire date 8-13-19.Plan of Correction: The director will ensure that staff #3 completes the training.
