The licensing inspector conducted an unannounced inspection on October 28, 2019 in response to a self-reported incident that the Department received on October 1, 2019 regarding a serious injury. The inspection was initiated on October 28, 2019 and completed on December 5, 2019. On October 28, the inspector was on site from 12:00pm to approximately 12:50pm. Interviews with staff were completed, observations were made in the center, and documentation was reviewed. An additional interview was conducted on November 6, 2019. The preponderance of the evidence gathered during the investigation did not support violations of the standards. There were no violations found during this inspection.
