63.2 Child Abuse & Neglect

Technical Assistance:
Discussion was held regarding information needed to complete staff and children's records.

Comments:
An unannounced monitoring inspection was conducted from 11:40 am to 5:15pm. At the time of entrance, 28 children were in care with 5 staff members present. Children were observed during lunch time and transitioning to nap. Diapering and hand washing procedures were observed. The site was clean, organized and contained a varied supply of developmentally appropriate materials. The staff was observed supervising children during lunch and bathroom breaks, supervising hand washing, cleaning the tables and diapering. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, required posted documents, medications and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-185-50-A
Description: Based on observation, personal information about children in care was not always treated confidentially.
Evidence:
1) In both the Butterflies and the SAK rooms, allergy lists were posted that included the names of children in care and their allergies. These lists were on plain sight, compromising the confidentiality of the Iinformation.

Plan of Correction: It was corrected during the inspection: allergy lists were covered.

Standard #: 22VAC40-185-70-A
Description: Based on record review, not all staff records contained all required information.
Evidence:
1) The record for Staff 3 did not contain a date of hire.
2) The records for Staff 1 - 4 did not contain the name, address and telephone number of a person to contact in case of an emergency, and information about health problems.

Plan of Correction: We will update the files by the end of the week.

Standard #: 22VAC40-185-240-A
Description: Based on record review and interview, staff did not receive all the required training by the end of their first day of assuming job responsibilities.
Evidence:
1) The records for Staff #1, #2 and #4 did not contain documentation that confirms that the Recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse training has been completed.
2) The director stated that they have not completed the training.

Plan of Correction: We will have the staff complete the training.

Standard #: 22VAC40-185-240-D-5
Description: Based on record review, it was determined that there was not always at least one staff member on duty who has obtained instruction in performing the daily health observation of children.
Evidence:
1) The records for Staff #1 - #4 did not contain documentation of having completed the daily health observation of children training.
2) The director stated that no other staff have completed the training.

Plan of Correction: I will complete the training tonight.

Standard #: 22VAC40-185-420-E-3
Description: Based on record review, not all children's record contained all the required information.
Evidence:
1) The records for Child 1 and Child 3 did not contain an annual parent confirmation that the required information in the child's record is up to date.

Plan of Correction: I will provide the parents with the forms and have them complete them

Standard #: 22VAC40-185-500-B
Description: Based on observation, it was determined that the diapering surface was not cleaned with soap and at least room temperature water and sanitized after each use.
Evidence:
1) In the caterpillar classroom, the diapering surface was sprayed with Lysol and wiped dry after each diaper change. By Licensing requirements, the surface must be cleaned and sanitized using a two-step process where the surface is cleaned with soap and water, wiped dry and then sprayed with a sanitizing solution and let air dry.

Plan of Correction: We will retrain our staff tomorrow.
