11/25 - Licensing Inspector arrived at 6:30 and departed at 7:45 AM to conduct a monitoring inspection. At the time of the inspection, there were 15 preschool and school-age children in care with three staff. Children were observed participating in arts & crafts activities and table-top toys. The sample size was five children's records.
12/2 - Licensing Inspector arrived at 11:30 AM and departed at 1:00 PM to conduct a staff record review. The sample size was four staff records.

Standard #: 22VAC40-185-140-A Description: Based on review of five children's records, it was determined that the facility did not ensure that each child had a physical on file within 30 days of enrollment.
Evidence:
The record for child #2 (date of enrollment 9/4/19) did not contain a completed physical examination.Plan of Correction: Site director will contact the parents to obtain a copy of the physical.
Standard #: 22VAC40-185-70-A Description: Based on a review of four staff records, it was determined that the facility did not ensure that all required elements were present in each staff member's record.
Evidence:
1. The record for staff #1 did not contain the two references as to character and reputation checked prior to employment or documentation of completing orientation to include hire date.
2. The record for staff #2 did not contain one of the two references as to character and reputation checked prior to employment.
3. The record for staff #3 did not contain one of the two references as to character and reputation checked prior to employment.Plan of Correction: District manager will ensure that all required elements are present in each staff member's record.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of four staff records, it was determined that the facility did not ensure that all staff submitted to fingerprinting prior to beginning employment.
Evidence:
The record for staff #3 did not contain documentation of completed fingerprints.Plan of Correction: District manager will obtain copy of fingerprints from the human resources office and place them in the record.
