Comments:
An unannounced monitoring inspection was conducted on December 6, 2019 from approximately 9:45am-1:00pm . Upon arrival to the facility, there were 15 children present with four staff members. The children were observed playing in centers, coloring and having free play. The infants were observed sitting and being fed. The sample size consisted of five children?s records and five staff records. The center does not administer medication. Areas of non-compliance were reviewed with Owner.

Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, in two of five children's records reviewed, the licensee did not ensure that each child shall have a physical examination before the child's attendance or within one month after attendance.

Evidence: The Owner confirmed that there was no documentation of a physical examination in the records for Child #1 (start date 9/4/18) and Child #3 (9/10/18).

Plan of Correction: The parent of Child #1 will be contacted to obtain a copy. The parent of Child #3 has already been contacted and it will be printed and placed in the record.

Standard #: 22VAC40-185-60-A
Description: Based on record review, in two of five children's records reviewed, the licensee did not ensure that each child's record contained all required information.

Evidence: The Owner confirmed the following information had not been documented in each child's record:

Child #3-There was no documentation of addresses for two persons to contact in the event of an emergency where the parent cannot be reached.

Child #5-There was not an employment phone number documented for one parent.

Plan of Correction: The parent for Child #3 arrived during the inspection and updated the paperwork. The record will be updated for the other child.

Standard #: 22VAC40-185-70-A
Description: Based on record review, in two of five staff records reviewed, the licensee did not ensure that each staff record included all required information.

Evidence: The Owner confirmed that there was no documentation that at least two references were checked prior to employment for Staff #3 and Staff #4.

Plan of Correction: The records will be updated.

Standard #: 22VAC40-185-270-A
Description: Based on observation and inspection of the facility, the licensee did not ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence: The Owner confirmed the following items observed at the center:

1. School Age classroom: The door knob leading into the classroom was becoming detached from the door. There was an area approximately 12'' long of blue peeling paint near the base board. There was a tall white storage cabinet where the door had become detached from three hinges on the left side and was hanging by a locked single hinge on the opposite side.

2. On the outdoor play area, there was a rod approximately 12" long attached to a wheel from a broken toy. There were several areas of exposed tarp posing a tripping hazard.

Plan of Correction: The broken toy was discarded and the doorknob was fixed during the inspection. The door will be removed from the hinges. The surface will be repainted. Owner will attempt to pull up the tarp.
