A Subsidy Health and Safety inspection was conducted on December 6, 2019 from approximately 9:45am-1:00pm to review supplemental health and safety requirements. Upon arrival to the facility, there were 15 children present with four staff members. The children were observed playing in centers, coloring and having free play. The infants were observed sitting and being fed. The sample size consisted of five children?s records and five staff records. The center does not administer medication. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-540-A Description: Based on record review, in one of five staff records reviewed, the vendor did not ensure that documentation of the communicable tuberculosis (TB) screening shall be submitted at the time of employment, prior to coming into contact with children and shall have been completed within the last 30 calendar days of the date of employment.
Evidence: The Vendor confirmed that the documentation of the TB screening for Staff #1 (date of hire 7/23/19) was more than 30 calendar days prior to the date of employment and was dated 3/13/19.Plan of Correction: Owner will make sure that in future hires it will be done within 30 days prior to their hire date.
