The licensing inspector conducted an unannounced monitoring inspection, at which play groups,activities, group time, personal hygiene, and lunchtime were observed. Children were observed; and staff and children files and other documentation were reviewed. Twenty-five (25) children, aged 6 months-5 years, were found to be in care with 5 staff. Previous violations were reviewed, and had been corrected. Please contact me if you need further assistance. Thank you for your cooperation. This inspection began at 9:30 am and concluded at 12:40 pm.

Description: Based on review of documents and discussions with staff the facility failed to maintain compliance with this standard as required relating to staff files.

The substitute that was working at the center during the inspection did not have a file available for review. The file had not been scanned to the center as of the close of the inspection.

Plan of Correction: Substitute files will be sent to each center so as compliance can be met in the future.

Description: Based on review of the facility and discussions with staff the facility failed to maintain compliance with this standard as required relating to physical plant.

There were 3 rooms that had equipment, materials, and other potential hazards stacked haphazardly within each room, as the center staff had been trying to update the facility by patching and painting the rooms. These 3 rooms were able to be accessed by the children in care as the doors did not lock.
Cans of paint were also in the gray room, accessible to the children.

Plan of Correction: Cooperate staff will be contacted and assistance requested for maintenance personnel to come and assist with needed upgrades so that upon opening each day the center will be fully ready to use and children will continue to be safe while in care.

Description: Based on review of staff files and discussions with staff the facility failed to maintain compliance with this standard as required relating to lead teachers.

There was no qualified lead teacher working with the 2 year old class during this inspection. No qualified lead for this class is currently employed. The staff who was in charge during the inspection will begin working toward qualifications for the position.

Plan of Correction: Staff are currently being trained for and obtaining training for the positions of lead teachers. We are still hiring for various positions, and all staff will become qualified for their positions if not already.

Description: Based on observation and discussion with staff the facility failed to maintain compliance with this standard as required relating to appropriate diaper disposal receptacles.

In the 2's classroom the foot operated diaper disposal can did not work. In order to lift the lid to dispose of a diaper, staff had to use their hand to open the lid, or leave the lid off completely while diapering.

Plan of Correction: A new foot operated can will be purchased. All disposal cans will be checked for working order and replaced or fixed to ensure compliance.
