63.2(17) License & Registration Procedures

Technical Assistance:
Provided consultation on "sanitizing" tables before and after each use for feeding. "Sanitizing" means treated in such a way to remove bacteria and viruses from surfaces through using a disinfectant solution. The surface of the item is sprayed or dipped into the disinfectant solution and allowed to air dry afterwards.

Comments:
An unannounced monitoring inspection was conducted from 11:45 am - 2:15 pm with the Falcon Club director. There were 36 children, ranging from three years to five years of age, supervised by six staff. Children were observed cleaning up, preparing to go outside, and playing outside on both the natural playscape and playground. Five children's files were reviewed and in compliance. Four staff files were reviewed. Three medications and authorization forms were reviewed. The last fire drill was conducted: 11/20/19 and the last shelter-in-place drill was conducted: 3/19/19. Standard #: 22VAC40-185-280-B
Description: Based on observation, not all hazardous substances were kept in a locked place. Evidence: there were Clorox disinfecting wipes on an open, top shelf in both of the pre-k 3's class bathrooms.

Plan of Correction: The hazardous substances will be moved to a locked cabinet.

Standard #: 22VAC40-185-280-D
Description: Based on observation, cleaning materials were stored above food. Evidence: there were three bottles of Lysol daily cleaner and disinfecting wipes located on a shelf above snack food items in the director's office.

Plan of Correction: The cleaning materials will be moved to a locked storage cabinet.

Standard #: 22VAC40-185-510-C
Description: Based on review of three medications, the center did not have written authorization from the child's physician and parent for medication to be administered. Evidence: there were no authorization forms for the medication for child #2 or child #3.

Plan of Correction: Will ask the parents to complete a medication authorization form.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on review of four staff records, it was determined that no Out of State Child Abuse and Neglect Search was requested by the end of the 30th day of employment. Evidence: Staff #2, who indicated that they have lived out of the state of Virginia in the past five years, had no documentation of an out of state search request in the file.

Plan of Correction: Will request an out of state search no later than December 20, 2019.
