Evidence:
1. The inspector reviewed five records. One of five (#5) did not have an immunization on file.Plan of Correction: Staff called the family service worker/family to get the record.
Standard #: 22VAC40-185-330-B Description: Based on measurement, the center did not maintain the required depth of resilient surfacing.
Evidence:
1. The inspector measured mulch under the slides and the climbing wall between 1-2 inches. Black plastic was exposed in 2-4 areas.Plan of Correction: Mulch will be redistributed and plastic cloth covered or removed.
