Comments:
The inspector conducted an unannounced mandated inspection from 10:45 until 11:30 at the main office where four staff records were reviewed and from 11:45-2:30 at the center. There were 10 children in care with staff supervising at all times. One medication was reviewed. Five children's records were reviewed. Children were observed at lunch, playing, during rest time and at circle time. Please call your inspector at 276-698-9981 with any questions.

Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not have an immunization record for a child enrolled.
Evidence:
1. The inspector reviewed five records. One of five (#5) did not have an immunization on file.

Plan of Correction: Staff called the family service worker/family to get the record.

Standard #: 22VAC40-185-330-B
Description: Based on measurement, the center did not maintain the required depth of resilient surfacing.
Evidence:
1. The inspector measured mulch under the slides and the climbing wall between 1-2 inches. Black plastic was exposed in 2-4 areas.

Plan of Correction: Mulch will be redistributed and plastic cloth covered or removed.
