Comments:
The inspector conducted an unannounced mandated inspection from 10:45 until 11:30 at the main office where four staff records were reviewed. The inspection continued at the center from 11:45 until 2:00. There were 12 children in care with staff supervising at all times. No medications were reported being given. Five children's record were reviewed. Children were observed playing, doing a craft, at lunch, washing hands,getting ready for nap time, and going to the bathroom. Description: Based on measurement, the center did not maintain the required depth of resilient surfacing.
Evidence:
1. The inspector measured the mulch and it was 1-2 inches under and around the slides and under the climbing wall (on the big piece of equipment). 2. Black plastic was exposed in three areas.

Plan of Correction: Mulch will be redistributed.
