Comments:
An unannounced complaint investigation inspection was initiated on 11/16/2019. The complaint investigation was in response to concerns regarding ratios in the Young Infant and Mobile Infant classrooms. Careful observations, attendance record reviews and staff attendance were made and interviews were conducted. The preponderance of evidence gathered during the investigation does not support the allegations, so the complaint is determined to be not valid. Description: Based on a review of children's records, the center failed to obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence:
Child #3's record does not have documentation of immunizations by the first date of attendance. First date of attendance is 10/17/19, proof of immunizations dated 11/12/19.

Description: Based on a review of children's records, the center failed to ensure that all children's records contained the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached;

Evidence:
1) Child #3's records did not contain the name, address and phone number of two designated people to call in an emergency if parents cannot be reached; the first emergency contact is the father.
2) Child#1's record does not contain the name, address and phone number of two designated people to call in an emergency if parents cannot be reached; there is only one emergency contact documented

Description: Based on a review of staff records, the center failed to ensure that staff records shall include written information to demonstrate that the individual possesses the education, certification and experience required by the job position.

Evidence:
1) Staff #1, #2 and #3's records do not contain documentation of education, certification or experience required by the job description.
2) Staff #4's record has a diploma as documentation of education required by the job description, but the record does not contain documentation of the translation of the diploma.
