Technical Assistance:
Technical assistance was provided regarding staff qualifications, reporting structures and lead teacher responsibilities.

Comments:
An unannounced monitoring inspection was conducted today from 9:30am-12:15pm. There were 136 children directly supervised by 23 staff. The physical plant, 10 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Infants were observed crawling freely, playing with bubbles, having tummy time and being held during bottle feedings. Toddlers were observed leaving the classroom, lining up and moving to music class. Preschoolers were independently using the bathroom, putting jackets on, reading books, and doing puzzles. The kindergarten calssroom was working in centers and the teacher was rotating through the centers working with children independently. Diapering and hand-washing procedures were also observed. There was a supply of materials for the children. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-185-50-A
Description: Based on observation, the center did not ensure children's records were kept confidential.

Evidence: A child's medication information was posted on the outside of an upper cabinet in the kitchen of the classroom. The information was available and not kept confidential.

Plan of Correction: We will move the information to an inside cabinet to keep it confidential.

Standard #: 22VAC40-185-90--A
Description: Based on record review, parents were not provided all required written agreements.

Evidence: Child 1 is missing information regarding communicable diseases and sick and medication policy.

Plan of Correction: We will have parents sign off at pick up today.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment inside the center were not kept in a safe condition.

Evidence: In the preschool 2 classroom, a lower cabinet door has a broken hinge and when opened could potentially cause a safety hazard to children.

Plan of Correction: We will send a ticket to our maintenance department to get repaired as soon as possible.

Standard #: 22VAC40-185-280-B
Description: Based on observation, all hazardous substances were not kept in locked place using a safe locking method that prevents access by children.

Evidence:
1) In early preschool classroom, there was an unlocked lower cabinet which contained cleaning products.
2) In the far back infant section, there was an unlocked lower cabinet which contained cleaning products.

Plan of Correction: We will move cleaning solutions to locked cabinets.

Standard #: 63.2(17)-1721.1-B-3
Description: Based on record review, out of state central registry background checks were not completed for all staff who have lived out of the state of Virginia within the last 5 years.

Evidence:
1) Staff 1 does not have a completed central registry background check and has lived out of the state of Virginia within the last 5 years. (Staff 1, DOH 7/29/2019)
2) Staff 2 does not have a completed central registry background check and has lived out of the state of Virginia within the last 5 years. (Staff 2, DOH 10/14/2019)

Plan of Correction: We will have staff complete and mail out both checks today.
