Comments:
An unannounced monitoring inspection was conducted on 12/6/19 from 10:20am until 12:00pm. At the time of entrance, there were 36 children in care with 7 staff and 1 administrator. A sample of 5 children's records and 5 staff records were reviewed. Children were observed participating in morning learning activities and eating snack. Parent pick-up procedures were also observed. Medication, first aid and emergency supplies, the emergency preparedness plan, documentation of emergency practice drills, children's injury reports and center postings were reviewed. Three violations in the areas of administration, physical plant and background checks are identified on the violation notice and were discussed with the center director during the exit interview.

Violations:
Standard #: 22VAC40-185-70-A
Description: Based on record review and interview, the center did not ensure that staff records contain all the required information.

Evidence:
1. The record for staff 1 (hire date: 8/26/19) does not contain documentation that two or more references as to character and reputation as well as competency were checked before employment.
2. The center director confirmed that the record for staff 1 lacked documentation of the required reference checks.

Plan of Correction: The center responded with the following: Reference requests were sent for staff 1 upon hire; however, they were not sent back. The center director will contact the provided references again and request the checks. If there is no response, staff 1 will be asked for additional references to contact.

Standard #: 22VAC40-185-260-B
Description: Based on a review of the most recent health inspection and interview, the center did not ensure that annual approval from the health department is provided.


Evidence:
1. Documentation of the most recent health inspection is dated 10/15/18.
2. The center director confirmed that the most recent health inspection is expired.

Plan of Correction: The center responded with the following: The health department was contacted before the last health inspection expired; however, they did not respond back. The health department was contacted again during the licensing inspection and they stated they will contact us on 12/9/19 to schedule the health inspection.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on record review and interview, the center did not ensure that staff who have lived outside of the state of Virginia within the past 5 years obtain the results of a child abuse and neglect registry search from that state.

Evidence:
1. Staff 1 (hire date: 8/26/19) indicated on her sworn statement or affirmation that she has resided in the state of Louisiana within the past five years. Results of a child abuse and neglect registry search were not obtained from Louisiana.
2. The center director confirmed that the results of a child abuse and neglect registry search were not obtained from the state of Louisiana for staff 1.

Plan of Correction: The center responded with the following: A child abuse and neglect registry search will be requested from Louisiana for staff 1. Sworn statements will be carefully reviewed to ensure that child abuse and neglect registry searches are obtained for any states in which staff have lived within the past five years.
