An unannounced monitoring inspection was conducted from 9:30 am-2:00pm. At the time of entrance, 53 children were in care with 10 staff members present. Children were observed during snack time, circle time, identifying colors, and playing independently. A selection of 4 staff and 5 children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. A subsidy health and safety inspection (SHSI) was also conducted during this inspection. Evidence:
1. Child #1 began attendance at the center on 7/11/17. The first documented record of immunizations available for review was dated 9/21/17.
2. Child #2 began attendance at the center on 12/11/17. The first documented record of immunizations available for review was dated 12/27/17.Plan of Correction: Immunizations were obtained and placed in the file. Children will not be permitted to begin until immunizations are turned in.

Child #1 began attendance on 7/11/17. The most recent documented parent confirmation that required information in the child's record is up to date was dated 7/1/18.Plan of Correction: We have a tracking system in place for children who's information needs updated. We will present the information to parents to be updated in a more timely manner.
Standard #: 22VAC40-185-70-A Description: Based on record review, 1 of 4 staff records did not contain all required information.
Evidence:
1. Staff #1 began employment on 11/25/19. The record for Staff #1 did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.Plan of Correction: We will make sure references are called as soon as we get their resume and application.
Standard #: 22VAC40-185-240-A Description: Based on record review, 1 of 4 staff did not receive all required training by the end of their first day.
Evidence:
Staff #1 began employment on 11/25/19. There is no documentation of training on recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse.Plan of Correction: We will make sure staff are aware they have to have the training completed before their first day.

Documentation of an evacuation drill for the month of November 2019 was not available for review.Plan of Correction: We will create reminders at the beginning of each month to complete the drills.
Standard #: 22VAC40-185-550-M Description: Based on record review, 1 of 4 injury reports did not contain all required information.
Evidence:
1 of 4 injury reports did not contain the time parents were notified.Plan of Correction: We will create a guideline for staff to make sure all necessary information is included.
