Comments:
An unannounced monitoring inspection was conducted today. There were 33 children in care, with a total of 7 staff members assigned to their direct supervision. Children's records, staff training records, background checks, new staff records, the physical plant, classroom programming, classroom supplies, administrative records, and emergency preparedness items were inspected today. It was reported to the Inspector that no children's medications were on site today.
The outdoor play area could not be inspected today due to current rainy conditions.

The children were observed coloring pictures, creating holiday art projects, enjoying circle time discussions, and eating lunch.
The children were served macaroni and cheese, oranges, salad, and milk for lunch today.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on a review of staff health records, it was determined that one staff person's TB screening was completed longer than 1 year prior to employment. In addition, there is no documentation of a completed TB screening for another staff person.
Evidence:
1 - Staff A began employment on 12/5/19. Her TB screening was completed on 11/29/17.
2 - Staff B began employment on 11/27/19. There is no documentation of a completed TB screening for this staff person.

Plan of Correction: I've already asked them for new TB tests. I will make sure they get them completed this week.

Standard #: 22VAC40-185-80-A
Description: Based on a review of administrative documents, it was determined that center staff have not kept a written record of children in attendance daily.
Evidence:
Of the 5 classrooms in operation today, 4 of them had not completed their daily written record of attendance.

Plan of Correction: All staff will be reminded to document attendance as children enter each morning.

Standard #: 22VAC40-185-240-A
Description: Based on a review of staff training documentation, it was determined that staff have not completed all required components of First Day Orientation Training.
Evidence:
Staff have not completed "Recognizing Abuse and Neglect" training, as required based on the fact that each staff person is a mandated reporter.

Plan of Correction: I was not aware that a separate, specific training was required for this. I train all staff on their roles as mandated reporters during orientation training. I will have all staff complete the online training course.

Standard #: 22VAC40-185-270-A
Description: Based on observations, it was determined that three areas of the school have not been maintained in a clean, safe and operable condition.
Evidence:
1 - In the Toddler classroom, the Inspector observed one child remove an electrical outlet cover from the outlet on the wall. This cover was too large and too loose for the outlet.
2 - In the Infant classroom, staff have placed a line of bouncey seats and one full size crib along one wall. These items have been placed in front of the classroom emergency exit door, thereby blocking the use of this door.
3 - Staff empty and rinse out the infant bottles of formula inside the classroom bathroom. The sink inside this bathroom is dirty.

Plan of Correction: 1 - I will purchase new outlet covers that fit our outlets properly.
2 - Staff will move the crib and bouncey seats immediately today.
3 - The bathroom sink will be cleaned.

Standard #: 22VAC40-185-280-B
Description: Based on observations, it was determined that various potentially hazardous substances were not stored in a locked place.
Evidence:
Cleaning products and containers of spray paint were stored inside unlocked cabinets and closets in the following classrooms: 3's and Infants

Plan of Correction: All items were immediately locked during the inspection.

Standard #: 22VAC40-185-280-G
Description: Based on observations, it was determined that substitute containers of cleaning products were not labeled to indicate the contents.
Evidence:
The spray bottles of mixed bleach/water solution were not labeled in the following classrooms:
Toddlers, 2's, Infants.

Plan of Correction: The bottles will be labeled today during nap time.

Standard #: 22VAC40-185-370-1
Description: Based on observations, it was determined that one infant was not moved to his assigned crib after falling asleep in a bouncey seat that was not designed to keep him comfortable for sleep.
Evidence:
In the Infant classroom, the Inspector observed one 12 month old child asleep while seated in a bouncey seat. Because this child is too tall for the seat, his feet were hanging down at the end of the seat. This child was not moved to his crib after falling asleep in the small bouncey seat.

Plan of Correction: The parent asked us to let him sleep in the seat because he is congested and the elevation helps him sleep. Instead of using the seats when children are congested, we will purchase sleep wedges to use with the crib mattress.
