Comments:
An unannounced monitoring inspection was completed today for the building located at 44821 Prentice Drive. The physical plant, 5 staff files, 10 children's files, annual fire and health inspection, staff training, child nutrition, children's activities, medications, emergency procedures, drills and supplies were inspected between 1:20-3:20pm. There were 71 children under the supervision of 13 staff. During the inspection, coloring, preparing for and eating lunch. For lunch, the children were served chicken lo mein with mixed vegetables, oranges and milk. The interactions between the children and staff were seen to be positive and constructive in nature. Description: Based on file review, 1 out of 5 staff records did not include all required documentation.
Evidence
1. Staff #4 did not have documentation that two or more references as to character and reputation as well as competency were checked before employment began.

Plan of Correction: There was a request for two references, but only one was able to be located during the inspection. If this cannot be found, staff #4 will be asked for a letter of reference.

Standard #: 22VAC40-185-290-3
Description: Based on observations, the center did not have protective covers on electrical outlets in areas used by preschool children (or younger).
Evidence
1. In the Owls classroom, there was an electrical outlet without a protective cover.

Plan of Correction: Corrected during the inspection.
