Comments:
An unannounced monitoring inspection and a self reported investigation related to sight and sound supervision were conducted today from 10:20am-1:30pm. There were 51 children (infants - 4 yrs.) directly supervised by 11 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, being bottle fed, napping, learning about nature, playing outside, and eating lunch. Today's lunch consisted of chicken crumble, whole grain spaghetti, fresh zucchini, pears and milk. Diapering and hand-washing procedures were also observed. There was an adequate supply of materials for the children. A follow-up visit was conducted on 11/13/2019 to complete the self-reported investigation. Based on the preponderance of evidence gathered, there were no violations cited related to the self-reported incident; however, areas of non-compliance not related to the self-reported incident are identified on the Violation Notice.

Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
Child#5's record contains an immunization record that was last updated on 2/14/2019. The immunization record was not updated every six months as required.

Description: Based on observation, it was determined that areas and equipment of the center, inside and outside were not maintained in a safe and operable condition.

Evidence:
1. In the preschool classroom there is a section of the wall next to the paper towel dispenser that is damaged. A soap dispenser was taken off the wall leaving a small hole and damaged drywall around it.
2. On the preschool playground the rubber playground surface was observed missing approximately 4 inches of the rubber material on the left side of the playground equipment.
3. On the preschool playground an area of approximately 4 inches in length of settling ground that presents a possible tripping hazard was observed next to the brick fence surrounding the A/C - heating units.
4. On the Preschool/Pre-K playground, multiple exposed tree roots protruded from the dirt, creating possible tripping hazards.
5. On the school age playground, areas of uneven ground were observed close to the back fence creating possible tripping hazards.
