An unannounced renewal inspection was conducted today from 1:30pm-4:30pm. There were 45 children ( 2yrs-5yrs.) directly supervised by 12 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed napping, washing hands, and eating afternoon snack. Today's snack consisted of boiled eggs, cantaloupe, and milk. There was an ample supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified on the Violation Notice.

Standard #: 22VAC40-185-60-A Description: Based on record review, children's records did not contain all the required information.
Evidence:
Child#2's record is missing the mother's place of employment and her work phone number. Child#4's record is missing the parents' work telephone numbers.Plan of Correction: I will ask the parents for the missing information.
Standard #: 22VAC40-185-70-A Description: Based on record review, staff records did not include all the required information.
Evidence:
Staff#1's record does not contain two references as to their character and reputation, as well as competency required prior to their first day of employment. The records of Staff#3 and Staff#5's record are missing documentation of orientation training and one reference required prior to their first day of employment.Plan of Correction: We will go over the missing information and have the staff member complete it.
Standard #: 22VAC40-185-290-3 Description: Based on observation, not all unused electrical outlets had protective covers.
Evidence:
Two outlets were observed in the middle room without protective covers. An outlet without a protective cover was observed next to the water fountain. A second outlet was observed without a protective cover next to the door that leads to the teacher's room.Plan of Correction: We will cover all the uncovered outlets, and we will assign a classroom teachers to take turns to monitor outlets every morning.
Standard #: 22VAC40-185-510-N Description: Based on review of medication and interview, medication with an expired authorization form was not returned to the parent within 14 days of expiration.
Evidence:
The medication authorization form for a prescription long term medication for child#6 expired on 9/9/2019. The medication was observed on-site during the inspection.Plan of Correction: We will return the medication to the parents today.
