Technical Assistance:
Consultation provided on policies and procedures- please submit polices and procedures for injury prevention and playground safety by 12/13/19. Please submit documentation of the daily health screening by 12/13/19. Please submit your revised Parent and Staff Handbooks by 12/12/19.

Comments:
An initial study was conducted today with the Administrative staff from 10 am to 2 pm. Many of the standards could not be inspected as there are no children enrolled at this time. The center was inspected and measured and the measurements supports a capacity of 17 children.There were ample supplies and age appropriate furnishings for the requested age range of 2 to 5 years.The playground was inspected today. The center will not be transporting children at this time.
Please provide a copy of your Emergency Preparedness Plan by 01/14/20.

Standard #: 22VAC40-185-40-I
Description: Based on interview, it was determined that there were no written procedures for injury prevention.

Plan of Correction: We will write the policies and submit by 12/10/19.

Standard #: 22VAC40-185-40-K
Description: Based on interview, it was determined that there was not written playground safety procedures.

Plan of Correction: We will write the policies and submit by 12/10/19.

Standard #: 22VAC40-185-330-B
Description: Based on observation and inspection, it was determined that there was not six inches of fall/use zone for playground equipment.
Evidence:
On the blue and red climbing structure there was not the required 6 feet of fall zone. The structure was within 3 feet of the wooden barrier on one side and the slide was within 4 feet 3 inches of the boarder on the other side.

Plan of Correction: We will remove this structure .

Standard #: 22VAC40-185-540-E
Description: Based on interview, it was determined that there was no battery operated radio available.

Plan of Correction: We will purchase a battery operated radio.
