Technical Assistance:
Licensing inspector discussed parental notification of medication administration with staff.

Comments:
An unannounced investigation was conducted 09/12/2019 in response to a self- reported incident regarding medication administration. During the investigation, staff were interviewed and staff and children's files were reviewed. The preponderance of evidence supports the allegation; therefore, violations were cited related to the self-reported incident. A violation not related to the reported incident was also cited. Please see violation notice for details. The exit interview was conducted with the director. Violations:
Standard #: 22VAC40-185-40-E
Description: Based on interview, the center failed to comply with their own policies and procedures.

Evidence:
1. On 09/04/2019, Staff A administered the medication of Child A to Child B.
2. Staff B stated she came into the office on the morning of 09/05/2019 and inquire about the medication of Child A that was left on the staff desk. Staff B stated after multiple staff, Staff A admitted that she administered the medication to Child B "because he had a fever"
3. Number 5 of subsection labeled "Stage 2-Dispensing and Administering Medication" of the Medication Policy states "No staff member may administer medication to any child other than the child for whom it is intended".
4. Staff A acknowledged receipt of this document 07/31/2019.

Plan of Correction: Staff A was terminated for violation of policy. All staff have been retrained on medication policy.

Standard #: 22VAC40-185-60-A
Description: Based on record review, A child's records did not contain required information.

Evidence:
The record of Child A was missing the address of one required emergency contact.

Plan of Correction: Retain missing information from parent.
