Technical Assistance:
Licensing inspector discussed staff emergency contacts with director.

Comments:
An unannounced monitoring inspection was conducted today from approximately 9am-10:45am. There were 60 children in the direct care and supervision of 18 staff. A sample of 5 children?s records, 5 staff records, 5 minor injury records, and 2 medications were reviewed. The children were observed singing happy birthday, making ornaments and making a holiday collage. Zero new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Description: Based on record review, each child did not have a physical exam.

Evidence:
Child A did not have a physical exam on file.

Plan of Correction: I will get that from the parents.

Standard #: 22VAC40-185-40-I
Description: Based on interview, the center did not develop written procedures for injury prevention.

Evidence:
Staff A stated she does not have this document.

Plan of Correction: I will develop a document to be updated annually. Corrected during inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place.

Evidence:
1. Disinfectant wipes, disinfectant spray, and sanitizing spray were found in an unlocked cabinet located in the Red Room.
2. Surface cleaner was found in an unlocked cabinet in the Yellow Room.

Plan of Correction: Corrected during inspection.

Description: Based on observation, hazardous substances were located above food.

Evidence:
1. Disinfectant wipes, disinfectant spray, and sanitizing spray were found in an unlocked cabinet above Goldfish crackers and other snacks in the Red Room.

The medications of Children B and C were kept in an unlocked cabinet in the main office.
