An unannounced monitoring inspection was conducted today from 12:00pm-2:35pm. There were 15 children directly supervised by 5 staff. The physical plant, 4 staff records, 4 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Per the director there were no medications on site. Children were observed participating in group play, playing outside, and eating snack. There an adequate supply of materials for the children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-185-40-B Description: Based on review of staff records it could not be verified that all who is or will be involved in the day-to-day operations of the center or who is or will be alone with, in control of, or supervising one or more of the children, shall be of good character and reputation and shall not be guilty of an offense.
Evidence:1. During inspection Staff A and Staff B did not successfully complete CPS background checks
2. Based on conversation during inspection there has not been an attempt made to obtain CPS checks from the other states where staff has resided.Plan of Correction: The forms were identified during inspection and director stated that they will be sent out.
Standard #: 22VAC40-185-60-A Description: Based on review of child records it was determined that all required information was not maintained.
Evidence: Child 1 was missing: Parent's workplace and phone number, and addresses for 2 of 2 emergency contacts and documentation of review.
Child 2 was missing: Parent's workplace and phone number, and address for 1 of 2 emergency contacts and documentation of review.
Child 3 is missing parents' work place phone numbers and documentation of annual review
Child 4 is missing: Parent's workplace and phone number, and documentation of review.Plan of Correction: I will check with the parents
