An unannounced monitoring inspection was conducted at the facility between the hours of 10 am and 12 pm. A sample of 2 children and 1 staff record was reviewed. There were no children with food allergies. Children were observed during circle time, playing toys and lining up for a bathroom break.

Standard #: 22VAC40-185-330-B Description: Based on observations the center did not ensure where playground equipment was provided, resilient surfacing complied with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 as shown in Figures 2.
Evidence:
There was a car-shaped spring rider on the playground that had about 4 inches of resilient surfacing underneath the piece of equipment and about 4 feet surrounding it. Six inches deep and 6 feet around is required.Plan of Correction: The following corrective action was provided by the director during the exit interview:
Additional resilient surfacing will be added to the spring rider to measure at least 6 inches deep and 6 feet around.
