Technical Assistance:
Discussion on staff records and physical plant. Discussed documents required to complete the renewal application.

Comments:
An unannounced monitoring inspection was conducted today approximately 10:40 am- 1:15 pm. There were 94 children supervised by 18 direct care staff. The physical plant, 7 staff records, 9 children?s records, children's nutrition, supplies, evacuation drills, injury reports, emergency supplies, and policies were inspected. Medications were on-site today and records were reviewed. The playground and surfacing were inspected. The children were observed in free play and in center activities. Diapering changing and hand washing was observed during the inspection. There were age appropriate materials for all children. Staff-to-child ratios were in compliance.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and furnishings of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1. In the infant 2 classroom, the shape an play mats had torn material that was accessible to children.
2. In the preschool classroom, a red chair had torn material that was accessible to children.

Plan of Correction: Will purchase replacments

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not ensure the diapering surface used only for diapering or cleaning children was cleaned with soap and at least room temperature water and sanitized after each use.
Evidence:
1. Staff #8 in the preschool 2?s classroom was observed changing a diaper.
2. After the diaper was changed, staff #8 only used soap and water to clean the diapering area.
3. Staff #8 did not sanitize the diaper surface after each use.

Plan of Correction: Will retrain staff that change diapers
