An unannounced monitoring inspection was conducted on 11/25/19 from 10:20am - 11:30am. During the inspection there were 26 children ages six months old through four years old in care with seven staff. Children were observed participating in various activities in the classrooms. Records were reviewed for six staff. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Standard #: 22VAC40-185-340-D Description: Based on interviews and a review of documentation, it was determined that the licensee did not ensure that in each grouping of children at least one staff member who meets the qualifications of a program leader or program director shall be regularly present.
Evidence:
1. Staff #1, staff #2, and staff #4 were working with 12 children in the Infant classroom.
2. The records for staff #1, staff #2, and staff #4 did not contain any documentation to demonstrate that any of the staff in the Infant classroom were Program Leader qualified.
3. Staff #6 (Program Director) confirmed that there was not any documentation available during the inspection to demonstrate that any of the staff in the Infant classroom were Program Leader qualified.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-185-350-E-2 Description: Based on observation and interviews, it was determined that the licensee did not ensure that for children, a ratio of 16 months old to two years, one staff member for every five children is maintained at all times children are in care.
Evidence:
1. There were 14 children present with staff #3 and staff #8 in the back classroom that is located beside the kitchen.
2. When staff #8 was asked how old the youngest child was, staff #8 stated that child #1 was 22 months old. Based on the age of child #1, the required staff-to child ratio would be one staff for every five children.
3. Staff #8 confirmed that the facility did not maintain the required staff-to-child ratio of 1 staff for every 5 children for this grouping of children.Plan of Correction: Not available online. Contact Inspector for more information.
Standard #: 22VAC40-185-500-A Description: Based on observation and interviews, it was determined that the licensee did not ensure that all children and staff wash their hands with soap and running water after a diaper change.
Evidence:
1. After a child in the Infant classroom had her diaper changed, staff #1 did not wash her hands before completing the next diaper change.
2. Two children who had their diaper changed in the Infant classroom were allowed to return to playing with toys without washing their hands.
3. Staff #1 confirmed that she did not wash her hands after completing a diaper change.
