Technical Assistance:
Technical assistance was provided in the following areas of the standards: 22VAC40-191 (Background checks); 22VAC40-185-(4)-270 (Building maintenance);-185-(4)-330 (Play areas);

Comments:
An unannounced monitoring inspection was conducted on 12/10/19 from 10:30am - 12:40pm. During the inspection there were 118 children ages six months old through five years old in care with 24 staff. Children were observed participating in various activities in the classroom, playing outside, eating lunch, and during rest period. Records were reviewed for ten children and 19 staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-510-G
Description: Based on a review of the medication being stored at the facility, it was determined that the facility did not ensure that in order to administer prescription medication, the center has obtained written authorization from a parent or guardian.

Evidence:
1. There was no written authorization for the medication (Epi-pen) for child #11.
2. Staff #20 (Program Director) confirmed that there was medication at the facility that did not have written authorization from the parent to be given.

Plan of Correction: The facility responded: The parent of child #11 will be asked to complete a new medication authorization. No medication will be accepted without a written medication authorization.

Standard #: 22VAC40-185-560-G
Description: Based on interviews, it was determined that the licensee did not ensure that when food is brought from home it is labeled with the child's name and date.


Evidence:
1. There were six bottles in the refrigerator int he Infant classroom that were not labeled with the date.
2. There were two lunch bags in the dining room that were not labeled with the date.
3. Staff #20 (Program Director) acknowledged that were bottles and lunch bags that were not labeled with the date.

Plan of Correction: The facility responded: Corrected during the visit by labeling the bottles and lunch bags with the dates. We will remind staff to label any food from home with the child's name and date.

Standard #: 22VAC40-185-570-K
Description: Based on observation and interview, it was determined that the licensee did not ensure that staff shall feed semisolid food with a spoon unless written instructions from a physician or physicians designee state differently.

Evidence:
1. There was a bottle for child #5 that had cereal mixed with the formula.
2. Staff 320 (Program Director) confirmed that there was no written instructions from a physician to feed cereal in the bottle to child #5.

Plan of Correction: The facility responded: We will talk to the parent and informed them that we can't feed a bottle with cereal in it unless there are written instructions from a physician.
