Comments:
An unannounced monitoring inspection was conducted between the hours of 8:00am and 11:00am. Upon arrival there were 40 children in care with 11 staff present. A sample of 5 staff and 4 children's records were reviewed. Children were observed eating snack, during handwashing, and circle time. Infants were observed in cribs and bouncers.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure areas and equipment of the center were maintained in a safe condition.
Evidence:
1. In the infant room there were 6 infant's observed sleeping in cribs with sheets that did not fit snug around the mattress.
2. There was peeling paint on the wall underneath the window in the infant room and in the infant/toddler hallway wall.

Plan of Correction: The following corrective action was provided by the owner during the exit interview:
The owner will look into mattress sheets in a portable crib size. In the meantime, the sheets will be fastened underneath the mattress to keep it snug.

Standard #: 22VAC40-185-280-B
Description: Based on observation the center did not ensure hazardous substances were kept in a locked place.
Evidence:
In the infant room there was a bottle of bleach water on the main counter top.

Plan of Correction: The following corrective action was provided by the owner during the exit interview:
The owner will speak to all staff about chemical storage.

Standard #: 22VAC40-185-420-E-1
Description: Based on observation and interview the center did not have documentation of the amount of time each infant who can not roll over, spent on their stomachs during "tummy time."
Evidence:
The log used to record an infant's day did not include time spent on their stomachs (tummy time). The staff stated they do provide tummy time, however it is not recorded or posted.

Plan of Correction: The following corrective action was provided by the owner during the exit interview:
The infant staff will begin noting tummy time on the infant logs.

Standard #: 22VAC40-185-440-J
Description: Based on observation the center did not ensure all crib requirements were met.
Evidence:
There were 3 infants observed sleeping in cribs that did not have at least 30 inches of space between occupied cribs and furniture. Due to equipment and furniture in front of the cribs, there was about 15 inches.

Plan of Correction: The following corrective action was provided by the owner during the exit interview:
The owner will speak to infant staff and rearrange the cribs and furniture in such a way that is in compliance with standards.

Standard #: 22VAC40-185-570-B
Description: Based on observation the center did not ensure all requirements for children with special feeding needs.
Evidence:
Two infants were observed sitting in bouncers, both drinking from bottles that were propped-up on rolled blankets.

Plan of Correction: The following corrective action was provided by the owner during the exit interview:
The regular infant staff were not in the room when this occurred. Training will be given to all staff on infant safety and how to properly feed bottle-fed infants.
