A subsidy health and safety inspection (SHSI) was conducted from 7:00 a.m. to 8:05 a.m. with the director and one staff. Three staff records were reviewed including staff training and CPR and first aid information. SHSI requirements were reviewed.

Standard #: 22VAC40-665-520-B-11 Description: Based on review of children's records and the allergy list, it was determined that the vendor did not obtain a written allergy care plan for diagnosed food allergies from a physician in the record. Evidence: Child #5 has a diagnosed food allergy to 'peanuts, tree nuts, sesame seeds, raw egg' and the allergy care plan is not signed by the child's physician. Plan of Correction: The allergy care plan will be followed up on with the parents and signed by the physican.
