Comments:
A mandated monitoring inspection was conducted today. There were 30 children present with 6 staff. Children we observed building with blocks, playing in sand, practicing their letters and sight words, playing house, washing hands, and napping. Reviewed 5 children's files, 5 staff files, Fire and Health inspection reports, medications and authorizations, attendance, menus, and physical plant. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on record review, each child did not have a physical examination by a physician within 30 days of enrollment.
Evidence:
Child#1 (start date 8/16/19) did not have documentation of a physical.

Plan of Correction: We will ask the parents for a copy of the physical

Standard #: 22VAC40-185-260-A
Description: Based on record review and interview with staff, the center did not provide to the licensing representatibe an annual fire inspection.
Evidence:
The last fire inspection available for review was dated 3/22/18.

Plan of Correction: I will call the Fire Department to get a copy of the last inspection.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous items were not kept in a locked place inaccessible to children.
Evidence:
Hand lotion labeled "Keep out of reach of children" and other potentially hazardous items were observed in an unlocked Teach cabinet in the 2 year old room.

Plan of Correction: We remind staff about locking cabinets that contain hazardous items

Description: Based on observation, electrical outlets did not have protective covers.
Evidence:
Electrical outlets in the 2 year old room were observed to be missing covers.

Plan of Correction: We will ensure cover are placed today

Standard #: 22VAC40-185-440-B
Description: Based on observation, cots were not identified for use by a specific child.
Evidence:
Cots throughout the center were not labeled to identify the specific child that uses it.

Plan of Correction: We will label all cots
