Comments:
An unannounced monitoring inspection was conducted today from 1:30am through 4:00pm. There were 66 children in care ages six months through ten years old with four teachers and three administrators. Children and staff records were reviewed. A walking of the building inside and outside was completed. Required postings, annual inspections, emergency evacuation drills, first aid kit, medication and transportation were reviewed. Children were observed in teacher and self-directed activities including restroom routines, hand washing play activities and nap. Violations regarding emergency equipment, cot spacing and sight supervision during nap were identified and appear on the violation notice. The violations were discussed with the program director assistant at the conclusion of the inspection.

Description: Based on observation and interview the center failed to ensure that children under 10 years of age always are within actual sight and sound supervision of staff.
Evidence:
1 - In the 4-6 year old room there are three children napping in areas of the room that are not visible to the staff supervising the group. The children are napping behind and between furnishing that obstruct the staffs' view.

2 - Staff 2 and 3 stated that they could not see all the children napping in their respective classrooms.

Description: Based on observation the center failed to ensure that there is 12 inches of space between occupied cots.
Evidence:
1 - In the 4-6 year old room there were five children napping on cots with less than 12 inches of space between them.
a - In the 3-4 year old room there were six children napping on cots with less than 12 inches of space between them.
2 - Staff 2 and 3 confirmed that there were children napping on cots with less than 12 inches of space between them.

Evidence:
1 - There is not a battery operated radio in building 2, the preschool building or building 3, the school age building.
2 - Staff 1 confirmed that there was not a working battery operated radio in building 2 or 3.
