Comments:
An unannounced monitoring inspection was conducted today, December 12, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at 9:45 am And concluded at 12:30 pm. The preschool children were observed playing in centers and later painting an art project. The two's class were having a sensory activity with colored water and different types of beans. Infants were observed watching a sing along video and the toddler/infants played with push toys and watched a video. Lunch time and nap time were also observed. All areas of the center were inspected. The first aid kit and non-medical supplies were inspected and found complete. The inspector interacted with staff and children in care.

Six children?s records and five employee record were reviewed during today's inspection.
Medication is not being administered.
Staff on site with current CPR, First aid and daily health observation training.
Most recent evacuation drill: 11/27/2019, shelter-in-place drills: 06/28/2019 and 07/29/2019. Lock down drill 07/29/2019.

Today's census:
Infants: 3 infants with 1 staff.
Infant/toddler: 5 children with 1 staff.
Two's: 8 children with 1 staff.
preschool (3-4 years) 11 children with 2 staff.

Violations cited during the licensing period were checked for corrections. There were no repeat violations found during today?s inspection. If you have any questions about this inspection, please contact the licensing inspector at 804-662-9758.

Violations:
Standard #: 22VAC40-185-350-C
Description: Based on observation and interview, the center did not ensure when children are in regularly in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group is applied.

Evidence:

Five children were observed in a classroom grouping with one staff. The younger child, child #7, was 15 months in age requiring a ratio of one staff member to every four children.

Plan of Correction: Per the owner:
A floater was available and should have been in the classroom, Moving forward I will make sure ratios are in compliance.

Standard #: 22VAC40-185-550-F
Description: Based on observation and interview, the center did not post a 911 or local dial number for police, fire and emergency medical services and the number of the regional poison control center in a visible place at each telephone.

Evidence:

1. There was no emergency numbers posted at the telephones of the center.
2, The owner stated the emergency numbers were not posted.

Plan of Correction: Per the owner: We moved a few things around and the emergency numbers were not posted. We will post them at the telephones.

Standard #: 22VAC40-185-560-F-4
Description: Based on observation and interview, the center did not ensure a menu listing foods to be served for meals and snacks during the current one week period was posted in a location conspicuous to parents.

Evidence:

1. A menu for the current week, 12/09/2019 was not posted. An outdated copy of a menu dated 11/25/2019 and 12/02/2019 was posted.
2. The owner acknowledged the current menu had not been posted.

Plan of Correction: Per the owner: I will post the current menu today.

Standard #: 22VAC40-185-570-A
Description: Based on observation, the center did not ensure when a child is placed in an infant seat or high chair, the protective belt was fastened securely.

Evidence:

1. Three children were observed in the infant room sitting in high chairs without the protective belt fastened.
2. Staff #4 acknowledge the three children were not fastened in the high chairs.

Plan of Correction: Per the owner: I will review the requirement to have the protective belts fastened in our staff training.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of staff records, the center did not obtain a completed central registry finding within 30 days of employment or volunteer service.

Evidence:

1. The record of staff #3, hired 11/11/2019, did not have documentation of a completed central registry exceeding 30 days from hire.
2. The owner stated the central registry finding had been requested on 11/20/2019 and had not been returned.

Plan of Correction: Per the owner: I will check on the status of the central registry form.
