Comments:
The inspector conducted an unannounced mandated renewal inspection from 11:00 a.m. until 12:40. There were 33 children in care with staff supervising at all times. The director and cook were also present. Children were observed being read to, playing at centers, washing hands, and eating lunch. Five children's records were reviewed and five staff records were reviewed as well as background checks for the licensee. Five staff records were reviewed. The playground was not inspected due to snow and ice coverage. The director stated that the playground violation from the last inspection had been corrected. An exit interview was held with the director and an acknowledgement form signed and left with the director. Please call your inspector at 276-698-9981 with any questions.

Description: Based on observation, the center did not keep all hazardous substances in a locked place.
evidence:
1. The inspector observed a can of Lysol on the top of a storage cabinet in the children's bathroom.

Plan of Correction: During the inspection, the director had a staff person lock the lysol up and put a sign on the cabinet door that all hazardous substances must be locked at all times and staff will be reminded to lock all hazardous substances at all times.
