Comments:
An unannounced monitoring inspection was conducted from 9:30am-11:45am. There were 33 children in the direct care and supervision of 10 staff. The children were observed having breakfast (brought in from home) and having circle time (counting and singing songs). A sample size of 5 children?s records, 4 staff records, evacuation drills, emergency materials, the first aid kit, and the physical plant were reviewed. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, all areas and equipment of the center were not maintained in a clean, safe and operable condition.
Evidence:
1. The gray sofa in the 4's classroom had a large rip in the cushion, exposing the material inside.

Plan of Correction: We will remove the sofa.

Standard #: 22VAC40-185-350-E-4
Description: Based on observation and staff interview, the center exceeded the 1:10 staff-to-children ratio requirement for children ages three years to five years.
Evidence:
1. The 4's classroom was observed to have 11 children (4 years old) with 1 teacher for approximately 20 minutes.
2. At approximately 9:45am, Staff A stated "I have 11 children and the second teacher will be arriving shortly."
3. At approximately 10:05am, the Director sent Staff B to the classroom to help support Staff A with the 11 children.

Plan of Correction: We have enough staff, but we will work on a better schedule to make sure we are always in ratio.
