Comments:
An unannounced monitoring inspection was conducted today from 9:35am to 2:00pm with the morning supervisor and the Director. There were 63 children in care, ranging from three months to five years of age, supervised by 11 staff. The children were observed during circle time discussing their weekly helper jobs and the calendar, practicing writing their letters and names, doing puzzles, painting, and having free play in centers. The playground was not observed due to rain. Six children?s records and five staff records were reviewed. The Center has staff with current certification in CPR and First Aid, as well as Daily Health Observation. There are reportedly no medications on site at this time, but the center has three staff current in Medication Administration Training (MAT). The attendance and emergency drill log were reviewed and were in compliance. The most recent Fire Inspection on file was dated 3/7/19 and the most recent Health Inspection on file was dated 10/2/19. Standard #: 22VAC40-185-130-A
Description: Based on review of five children's records, the center did not obtain documentation that each child had received the immunizations required by the State Board of Health before the child attended the center. Evidence: Child B, with a start date of 10/14/19, did not have documentation of immunizations in their record.

Plan of Correction: I will either locate the immunizations or ask the parents for another copy.

Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records, the center did not obtain documentation of a physical examination by a physician within one month of attendance. Evidence: Child B, with a start date of 10/14/19, did not have documentation of a physical in their record; and Child C, with a start date of 7/3/19, did not have documentation of a physical in their record.

Plan of Correction: I will obtain these from the parents.

Standard #: 22VAC40-185-160-C
Description: Based on review of five staff records, the center did not ensure that staff obtained and submitted the results of a follow-up tuberculosis (TB) screening at least every two years from the date of the first initial screening or testing. Evidence: The most recent documentation of a TB test in the record for Staff C was dated 7/21/17.

Plan of Correction: The staff will get this done after work today.

Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, the center did not obtain all required documentation for each child's record. Evidence: The records for Child B and Child C did not contain addresses for two emergency contacts.

Plan of Correction: I will get the information from the parents.

Standard #: 22VAC40-185-70-A
Description: Based on review of five staff records, the center did not have documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: Staff A, with a start date of 5/3/19, did not have documentation of a second reference in their record.

Plan of Correction: I will get that today.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances, such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the three year old classroom there was a spray bottle of bleach and water and an aerosol sanitizing spray in an unlocked upper cabinet. 2. In the two year old classroom there was a spray bottle of bleach and water, a spray bottle of multipurpose cleaner, and an aerosol sanitizing spray in an unlocked cabinet over the changing table. 3. In the Infant classroom there was a container of antibacterial wipes in an unlocked cabinet under the sink.

Plan of Correction: I spoke with the staff about this during the inspection and it was corrected.

Standard #: 22VAC40-191-60-C-2
Description: Based on review of five staff records, the center did not obtain documentation of the results of a central registry check within 30 days of employment for each staff. Evidence: The records for Staff B, with a start date of 10/7/19, and Staff C, with a start date of 7/20/17, did not contain documentation of the results of a central registry check.

Plan of Correction: I am sure I had the results for Staff C, so I called to request a copy from the central registry office during the inspection and had to leave a message. I will also follow up on Staff B when they call me back. If I do not receive the results from them today, I will not schedule them to work until the results have been received.
