Comments:
A mandated monitoring inspection was conducted today. There were 87 children present with 14 staff. Children were observed having lunch (quesadilla, carrots, pears and milk), having tummy time and napping, having show and tell, reading a book in circle time and playing the Freeze dancing game. Reviewed 7 staff files, 2 children's files, fire and health inspections, attendance, menus, medications and authorizations and physical plant. Violations:
Standard #: 22VAC40-185-90--A
Description: Based on record review, a written agreement between the parent and center stating the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any re-portable communicable disease has not been obtained,
Evidence:
Child#1 and Child#2 do not have signed written agreement the parents will notify the center should their child or someone in the immediate household develops a communicable disease within 24 hours.

Plan of Correction: I will alert my supervisor to the missing policy- and in the meantime, we will create an addendum to give to parents for signature.

Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment in the facility are not being maintained in a clean, operable and safe manner.
Evidence:
1. Observed frayed soft play equipment in the Toddler A to have exposed threads, stuffing and broken plastic which is a hazard.
2. Observed peeling and broken dry wall in the Beginners, Intermediate, PreK-1 rooms.
3. Observed splattered food on walls, doors and on lids of garbage cans in the Beginners, Intermediate, and PreK-1 rooms.
4. Observed dirt, grime and debris on walls in Beginners, Intermediate and PreK-1 rooms.
5. Observed missing or unsecured ceiling tiles in the Beginners and Pre-K2 rooms.
6. Observed rust and pitted paint accessible to children on the dividers of the bathroom in the PreK-.1

Plan of Correction: A work order will be placed to address the issues.

Standard #: 22VAC40-185-500-B
Description: Based on observation, a nonabsorbent surface was not being used for diapering or changing.
Evidence:
The diaper changing pad being used in Toddler A has a slice in the plastic covering, exposing the internal stuffing.

Plan of Correction: I will order a new one.

Standard #: 22VAC40-185-570-B
Description: Based on observation, bottle fed infants who cannot hold their own bottles were not being held when fed.
Evidence:
Child#1 was observed laying down on a boppy pillow while being fed a bottle.

Plan of Correction: We will re-train staff on proper feeding procedures
