Comments:
An unannounced monitoring inspection was conducted December 10 2019 between 10:15am and 12:15pm. There were 43 children in the direct care of 7 staff members. Children were observed singing, dancing, completing art projects, writing letters, practicing numbers, and playing with toys. Staff and child records were reviewed. Classrooms were inspected, and ratios were observed to be maintained throughout. Areas of non-compliance are listed on the violation notice. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not ensure that a child record had documentation of immunization before the child can attend the center.

Evidence: Child #2 is in care, but the child's record does not contain an immunization record.

Plan of Correction: I will have the parents bring the forms in.

Standard #: 22VAC40-185-60-A
Description: Based on record review, 1 of 4 child records reviewed did not have the required emergency contact information.

Evidence: Child # 3 record has 2 documented emergency contacts, but the full addresses are not listed for the contacts.

Plan of Correction: I will ask the parents to fill it out.

Standard #: 22VAC40-185-90--A
Description: Based on record review, 2 of 4 children's records did not contain documentation of a written agreement between the parent and the center by the first day of the child's attendance.

Evidence:Authorization for emergency medical care was not completed for Child #2 and Child #4

Plan of Correction: I will contact the parents and have them sign it.

Standard #: 22VAC40-185-510-A
Description: Based on observation, the center does not have the proper authorization to administer emergency medication to children.

Evidence: The medication authorization form gives authorization for medication to be administered by Fairfax County Public Schools and School Age Child Care. The form does not give authorization to Elim Kids Acadmey.

Plan of Correction: I will get the correct form and get the parents to take it to the doctor for signatures.
